Intake forms
Your own patient intake form, filled in before the patient reaches the desk.
Design the registration form once. Patients complete it as they book online, staff use the same form at the desk, and every answer lands on the patient record.
Every clinic asks different questions at registration. In MedOS a workspace admin builds the patient form from the settings: add sections, notices and questions, and rename or hide the standard fields the clinic does not need. Fields include short and long text, numbers, dates, dropdowns, yes/no, checkbox groups, ratings, email and phone, terms acceptance, a signature and file uploads.
The form shows in the online booking flow, so a patient fills it in on their phone before the visit, and in the add-patient and edit-patient screens the desk uses. A preview shows exactly what the patient will see. Fields marked sensitive are stored encrypted.

Works with
- Online booking
A patient books from your website, a shared link or a QR at the door. The booking lands in the upcoming column of the board with nothing re-typed at the desk.
Read more - Patient records
Visits, last seen, next appointment, outstanding balance, notes, prescriptions and activity, on one record. Nothing is re-entered at the desk.
Read more - Self check-in
Patients verify who they are and book a slot or join the queue from their own phone. They are on the board before they reach the desk. Nobody re-enters a name.
